Need some tips on gratuities

Hi,
I've found some info on tipping by using the search bar at the top of the forum. To summarize, it seems that tipping around 10 % for good service is the bar, however, this came from an older post ~ 2002. Also, tipping should be done discretely, so as to not insult whomever your tipping. Hotel cleaning staff should be tipped at the end of your stay (I don't agree with this if you have multiple people servicing your room). Also, what if your staying at a B and B, do you tip the proprietor? What about groundskeepers, security guards? I know this is a sticky subject for some.
