I'm sure that Steve W would have answered your questions by now, but as he's in Tobago and his internet connection is intermitent - I'll do my best.
1. A two-pin US style adaptor (same as those used in Barbados) will be fine.
2. There are several small local bars, but for some reason or other, I can never remember their names when I come out of them
Black Rock has a couple of bars (I think Michael's is on the main road) and it is a short stroll to Pleasant prospect (past Le Grand Courland hotel) where the sportsman bar / or late-comers as I think it's now called, is situated above the pizza boys shop.
3. This is also the closest ATM to Black Rock (right next to pizza boys) see the pictures and comments at

Blue Machine ATM looks like a phone kiosk in the pizza boys picture.
4. Mmmm - choice for dining out is a very personal thing.
If you are looking for a nice restaurant, within walking distance of Back Rock, then I would suggest either the
Sea Horse Inn ( 5 mins along the Grafton beach road), or
Indigo (at pleasant prospect, turn into the side road on the left at the little yellow 'Sunrise tours' office).
The Pavillion at The Stonehaven Sanctuary Villas would also be on my list of special places to dine in the Black Rock area.
You should check whether a reservation is required and book earlier in your stay.
